Tell Us About Your Camper (Required)

This Health Form is required for all campers

This Physical Exam Form is required if your camper is a:

• Resident Camper attending Chestnut Ridge for the first time
• Resident Camper whose doctor's signature on a previous year's form is for a physical exam done more than 24 months prior to camp
• Day Camper going off-site on the Rock & Raft trip
