Tell Us About Your Camper (Required for all campers)

Health Form (Required for all campers)

In addition to the Health Form above, the Physical Exam Form is required if your camper is a:

• Resident Camper attending Chestnut Ridge for the first time...or
• Resident Camper whose doctor's signature on a previous year's form is for a physical exam done more than 24 months prior to camp...or
• Day Camper participating in an off-site program (e.g. the Rock & Raft trip)
